Setting up Government Funding - the Government Payment Form
The Government Payment form is required by many of our providers so that they will pay Mycare directly.
To use your Government Funding directly on Mycare, there are many formalities that must be followed which are directed by your Funding Provider.
One of the requirements for a provider to pay Mycare directly, is that they receive authorisation from you that they may pay your funding directly to Mycare (note this is a general authorisation, and they will always seek your approval for each amount paid).
Before Mycare can approve you as a Government Funded client, we need confirmation that this authorisation has been given to your funder.
Please download the form, and email it directly to your funder, as well as copying Mycare hello@mycare.co.nz

To: Please write your IF Host name
Person Receiving Funding Name: The name of the person who has been allocated the funding.
Agent Name: The agent is the person that deals with the IF host regarding all aspects of funding/payments. This may be the person receiving funding, or a family member/nominated person who deals with the IF Host on their behalf. Note the Mycare account organiser should be the same person.
Please note that Government Funding will only pay in arrears. Mycare will pay your workers weekly in good faith, while awaiting your Government Funding provider to pay us after the fact. Because of this, it's really important that Mycare knows the payment authorisation form above has been sent, and also that you have budget remaining to pay your workers.
If for some reason your funder won't pay us, you will be responsible for paying Mycare for any visits made.
